¡Hola! Are you ready for an exciting opportunity? As a Licensed Practical Nurse (LPN), you’ll be an essential part of our team, reporting to the Program Director and Nurse Practitioner. Your role involves ensuring the well-being of our residents, including assessing their health needs, developing care plans, and maintaining medical records.
Essential Functions:
- Get ready to dive into patient care! From obtaining medical histories to administering medications, your role will be dynamic and hands-on.
- Educate and empower our residents on various health topics, tailored to their needs and in line with agency and individual care plans.
- Keep a watchful eye on residents’ physical and behavioral changes, ensuring prompt reporting and action.
- Be prepared to respond to medical emergencies and assist with medical appointments and transportation as needed.
- Join us in promoting health education programs for youth and workshops for staff, contributing to a supportive and informed environment.
Job Requirements:
- Knowledge: Stay up-to-date on policies, nursing protocols, and relevant health regulations to ensure effective program operations.
- Physical Skills and Abilities: Be ready to move! Your role may require extended computer use, accessing various locations, and even operating a motor vehicle.
- Mental Skills and Abilities: Your bilingual skills in English and Spanish will be invaluable in building rapport with our diverse community. You’ll also need strong communication, crisis management, and documentation skills.
- Working Environment: Embrace the diverse settings of our office and clinical environments, where you’ll encounter infectious diseases and individuals with trauma histories.
- Education/Experience: A bachelor’s degree and at least two years’ experience as an LPN are required, with previous experience in pediatric or adolescent care preferred.
- Licenses/Certifications: Hold a current NYS License and Registration as a Practical Nurse, along with Therapeutic Crisis Intervention certification and a valid NYS Driver’s License.
